Campaign Incentive Sweepstakes

1. SPONSOR. The 2025 United Performing Arts Fund Campaign Incentive Sweepstakes is sponsored by United Performing Arts Fund, Inc. (“Sponsor”), 301 W. Wisconsin Avenue Suite 600, Milwaukee, WI 53203.

2. ELIGIBILITY. NO DONATION OR PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW. MAKING A DONATION OR PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. The 2025 United Performing Arts Fund Campaign Incentive Sweepstakes (“Sweepstakes”) is open to natural persons who are legal residents of the State of Wisconsin and eighteen (18) years of age or older at the time of entering the Sweepstakes. Employees of United Performing Arts Fund, Inc. and members of their immediate families (spouse, children, and in-laws) are not eligible to enter the Sweepstakes.

3. TO ENTER SWEEPSTAKES. The Sweepstakes begins on September 1, 2024 at 12:01 a.m. Central Time and ends on June 30, 2025 at 11:59 p.m. Central Time (“Sweepstakes Period”). Each eligible person is entitled to one (1) Sweepstakes entry during the Sweepstakes Period. There are four (4) ways to enter the Sweepstakes:

   1. an eligible person who increases, or submits a pledge form to United Performing Arts Fund, Inc. to increase, his or her total 2025 gift to United Performing Arts Fund, Inc. by One Hundred and Fifty Dollars ($150) or more, or gives a new gift of One-Hundred and Fifty Dollars ($150) or more will be automatically entered in the Sweepstakes by the Sponsor for one chance to win a backstage dinner for ten (10) at the Miller High Life Theater, four (4) tickets to the U.S. Bank Founders Suite for a Milwaukee Brewers game, or a six (6) performance subscription for two (2) to the Milwaukee Symphony Orchestra including a tour of the Bradley Symphony Center, which are described below; or

       

   2. an eligible person who increases, or submits a pledge form to United Performing Arts Fund, Inc. to increase, his or her total 2025 gift to United Performing Arts Fund, Inc. by Seventy-Five ($75) or more, or gives a new gift of Seventy-Five Dollars ($75) or more will be automatically entered in the Sweepstakes by the Sponsor for one chance to win a Grand Geneva prize package, four (4) tickets to a Green Bay Packers game, four (4) premium tickets to a Milwaukee Brewers game, or four (4) tickets to a Skylight performance including a backstage tour, which are described below; or

       

   3. an eligible person who increases, or submits a pledge form to United Performing Arts Fund, Inc. to increase, his or her total 2025 gift to United Performing Arts Fund, Inc. by Fifty Dollars ($50) or more, or gives a new gift of Fifty Dollars ($50) or more will be automatically entered in the Sweepstakes by the Sponsor for one chance to win a PNC Major League Moment Package, a subscription for two (2) to Danceworks performances, four (4) holiday performance tickets to either the Milwaukee Ballet's Nutcracker or the Milwaukee Rep's Christmas Carol, or four (4) UPAF performance vouchers, which are described below; or

       

   4. an eligible person who does not make a gift to United Performing Arts Fund, Inc. and/or does not otherwise qualify to be automatically entered in the Sweepstakes by the Sponsor may enter the Sweepstakes for one chance to win each of the prizes described below by legibly printing his or her name, address, telephone number, and the prize(s) for which he or she would like to be entered to win on a sheet of paper and mailing it to United Performing Arts Fund, Inc., Attn: Campaign Drawing, 301 W. Wisconsin Avenue, Suite 600, Milwaukee, WI 53203.

       

   All pledge forms and mailed entries must be received by the Sponsor before the end of the Sweepstakes Period. Limit one entry per person per prize, regardless of the method of entry. In the case of the automatic entries (as described in (a) through (c) above), the Sponsor will enter the eligible person in each of the prize drawings for which he or she is eligible. Sponsor assumes no responsibility for any lost, late, stolen, incomplete, illegible, inaccurate, undelivered, delayed, mutilated, postage-due or misdirected mail, pledge forms, entries, correspondence, requests, claims or other errors or problems in connection with or relating to the Sweepstakes.

4. PRIZES.  Eleven (11) total prizes will be awarded:

• Backstage dinner for ten (10) at the Miller High Life Theater. One (1) winner will be drawn. Date to be mutually agreed upon by both parties. Verifiable retail value: $2,500.
• One (1) six (6)-performance subscription for two (2) to the Milwaukee Symphony Orchestra and a tour of the Bradley Symphony Center with the MSO's Executive Director. Two (2) winners will be drawn. Choice of Pops or Classics Series. Verifiable retail value: $1,152.
• Grand Geneva prize package. One (1) winner will be drawn. One-night stay at the Grand Geneva resort with golf and dining certificates. Date to be mutually agreed upon. Verifiable retail value: $599.
• Subscription for two (2) to Danceworks performances. One (1) winner will be drawn. Verifiable retail value $599.
• PNC Major League Moment. One (1) winner will be drawn. Includes four (4) tickets, autographed baseball, swag, and an on-field experience for one (1) child. Date to be mutually agreed upon. Verifiable retail value: $580.
• Holiday performance package. One (1) winner will be drawn. Includes four (4) tickets to either the Milwaukee Ballet's Nutcracker or Milwaukee Rep's A Christmas Carol. Verifiable retail value $550.
• Four (4) tickets to the U.S. Bank Founders Suite on Saturday, September 27 for a Milwaukee Brewers game. One (1) winner will be drawn. Includes parking, food, and beverages. Verifiable retail value: $500.
• Four (4) tickets to a Green Bay Packers game. One (1) winner will be drawn. Game to be mutually agreed upon by both parties. Milwaukee season ticket package only. Verifiable retail value: $500.
• Four (4) premium tickets to a Milwaukee Brewers game. One (1) winner will be drawn. First base side from the Brewers dugout. Game to be mutually agreed upon by both parties. Verifiable retail value: $425.
• Four (4) tickets to a Skylight Music Theater performance of your choice and a backstage tour with the Executive Director. Performance to be mutually agreed upon by both parties. One (1) winner will be drawn.  Verifiable retail value: $400
• Four (4) UPAF Performance Vouchers. One (1) winner will be drawn. Verifiable retail value: $300.

• All applicable taxes and other expenses associated with the acceptance and/or use of the prizes are the sole responsibility of the winner.

5. DRAWINGS. The winner of each prize will be selected from among all eligible entries for that prize in a random drawing to be held at the United Performing Arts Fund offices, 301 W. Wisconsin Avenue Suite 600, Milwaukee, WI 53203, on July 8, 2025. Limit one prize per person. Upon winning a prize, the winner will not be eligible to win a subsequent drawing for a prize. Winners need not be present to win. Selected entrants will be notified by Sponsor by telephone, mail and/or at an email address provided at the time of entry at Sponsor’s sole discretion. If a selected entrant does not respond to the Sponsor’s prize notification within five (5) days of the first notification attempt, then the selected entrant will be disqualified and an alternate entrant will be selected. No cash equivalent or substitution of prizes is offered, except in the sole discretion of the Sponsor. Prizes are non-transferrable, non-refundable and have no cash value. The odds of winning depend on the number of eligible entries received during the Sweepstakes Period.

6. GENERAL CONDITIONS. Entrants and winners agree to comply with these official rules and the decisions of the Sponsor. The Sponsor’s decisions, which are based on its sole discretion and interpretation of these official rules, are final and binding in all respects. The Sponsor reserves the right to cancel, terminate or modify this Sweepstakes if the Sweepstakes cannot be conducted as planned. Entrants and winners grant the Sponsor, and those acting pursuant to Sponsor’s direction, the right to use entrant’s and winner’s name, photograph, likeness, statement, image and/or recorded audio or video footage, for trade, advertising, publicity and promotional purposes in perpetuity without compensation (unless prohibited by law) in all media now known or hereinafter discovered, worldwide, including, but not limited to, the world wide web, without notice, review or approval, and agrees to execute specific consent to such use or uses if requested. Winners will be required to complete an affidavit of eligibility, liability release and publicity release, along with an Internal Revenue Service Form W-9, which must be returned within the time specified by the Sponsor. All applicable taxes, transfer fees and expenses associated with the acceptance and use of the prizes are the sole responsibility of the winner. Prize value may be reported as taxable income to the winner. Participation in this Sweepstakes may affect the tax deductibility of your gift. You should consult your tax advisor for more detailed information.

7. NO LIABILITY. In no event will the Sponsor, their respective advertising and promotion agencies, parents, subsidiaries, affiliates, shareholders, members, agents, representatives, dealers, employees, directors and/or officers (collectively, the “Sweepstakes Parties”) be responsible or liable for any damages or losses of any kind, whether direct, indirect, incidental, consequential, punitive or other damages arising from or in connection with this Sweepstakes. BY PARTICIPATING IN THIS SWEEPSTAKES, ENTRANTS AND WINNERS AGREE TO HOLD HARMLESS THE SWEEPSTAKES PARTIES FROM AND AGAINST ANY AND ALL CLAIMS OR CAUSES OF ACTION ARISING OUT OF PARTICIPATION IN THE SWEEPSTAKES OR RECEIPT, USE OR MISUSE OF ANY PRIZE, INCLUDING PROPERTY DAMAGE, PERSONAL INJURY OR DEATH. ENTRANTS AND WINNERS FURTHER AGREE THAT IN ANY CAUSE OF ACTION, THE SWEEPSTAKES PARTIES’ LIABILITY WILL BE LIMITED TO THE COST OF PARTICIPATING IN THE SWEEPSTAKES, AND IN NO EVENT SHALL THE SWEEPSTAKES PARTIES BE LIABLE FOR ATTORNEYS FEES, PUNITIVE, CONSEQUENTIAL, DIRECT OR INDIRECT DAMAGES. ALL PRIZES ARE PROVIDED “AS IS.”
